Primary activities relating to ACT Assess & Connect 

Dr. Warner is a primary research investigator in the ACTing Collectively project in Nova Scotia. The aim of this project is to assess older adults’ unmet needs in communities using the ACT Assess & Connect tool.

Dr. Warner is involved in a range of research projects with health system administrators and providers to inform and facilitate implementation of innovations to improve the lives of older adults and their families. Projects include remote monitoring of home care clients, assessing frailty in primary healthcare, self-management strategies for older adults, and a palliative approach to care.
